This form is only a “request for availability” and is reviewed by Carol Stream Park District Staff before final rental availability is confirmed.
This request is to be submitted at least 14 days prior (not to exceed 3 months prior) to the date requested. Once approved, a Rental Contract is generated and full payment is due within seven (7) days. It is only upon execution of the contract and receipt of payment that the rental request is guaranteed.
